Request to be Selected as Payee - SSA requires the information collected by the form SSA-11-BK as part of its investigation of a potential payee for a Social Security, Supplemental Security, Black Lung or SVB beneficiary. These investigations obtain information about a payee applicant's relationship to the beneficiary, personal qualifications, how concern for the beneficiary's well-being is demonstrated and how the payee applicant intends to use benefits if appointed as payee.
